Splenic flexure volvulus is a rare condition often associated with colonic fixation anomalies. A case of splenic flexure volvulus following surgery for sigmoid volvulus is reported. The patient, an 87-year-old man, underwent sigmoidectomy for sigmoid volvulus two years earlier. He presented to the hospital with complaints of abdominal distension, and computed tomography showed splenic flexure volvulus. Endoscopic detorsion was performed on the same day, and elective surgery was planned for one month later. Resection of the redundant colon including the splenic flexure and reconstruction with an end-to-end anastomosis were performed. During the surgery, a mobile cecum was noted, and colopexy was performed. The patient's postoperative course was uneventful. We recommend inspecting the entire colon and, if necessary, performing colopexy during surgery for splenic flexure volvulus.
