Aortic aneurysm rupture is a lethal disease. One of the most common causes of aortic aneurysm is arteriosclerosis. We report an extremely rare case of undifferentiated pleomorphic sarcoma incidentally found with impending rupture of the descending thoracic aortic aneurysm. The patient was a 50-year-old woman who had a backache. Urgent replacement of the descending aorta was performed due to impeding rupture of the descending thoracic aortic aneurysm. Enhanced CT scan and intraoperative findings showed wall irregularity of the descending thoracic aorta. The histological diagnosis was undifferentiated pleomorphic sarcoma. 20 days after surgery, we performed resection surgery for mediastinal tumors. The postoperative course was uneventful, and the patient was discharged 30 days after first surgery.
