Significance of peritoneal cytology on the prognosis of patients with primary uterine endometrial adenocarcinoma from the viewpoint of the cellular proliferative activity

Abstract
Objective : The aim of this study was to clarify the correlation between the tumor cell proliferative activity in ascites and the prognosis of patients with primary uterine body adenocarcinoma.
Study Design : For the analysis, 77 cases of primary uterine body adenocarcinoma without extrauterine metastasis, including 31 cases with positive peritoneal cytology and 46 cases with negative cytology, were examined. To evaluate the tumor cell proliferative activity, double-immunohistochemical staining for MIB-1 and MOC31 was performed.
Results : The MIB-1 expression ratio of the tumor cells in the ascitic fluid ; type 1 endometrial carcinoma (n=20), average 1.4% ; type 2 endometrial carcinoma (n=11), average 16.1%. The histological MIB-1 expression ratio in the primary lesions : type 1 endometrial carcinoma, 35.0% ; type 2 endometrial carcinoma, 57.1%.
Five-year survival ratio of the patients with type 1 carcinoma : patients with positive peritoneal cytology, 85.6% ; patients with a negative cytology, 96.0%. Five-year survival ratio of the patients with type 2 carcinoma : patients with a positive peritoneal fluid cytology, 50.9% ; patients with a negative cytology, 64.2%.
Conclusion : The results suggested that most of the tumor cells in ascetic fluid may regress naturally in cases of uterine adenocarcinoma without extrauterine invasion or metastasis.
