A case of undifferentiated pleomorphic sarcoma with giant cells arising from the chest wall with pleural effusion

Abstract
Background : Undifferentiated pleomorphic sarcomas arise principally in the extremities or retroperitoneum in adults. We report a case of undifferentiated pleomorphic sarcoma with giant cells arising from the chest wall.
Case : A 70-year-old man was admitted to our hospital because of left-sided back pain. Chest X-ray revealed a rapidly growing tumor located in the left chest wall. Aspiration fluid of the tumor and pleural fluid cytology showed oval to spindle-shaped cells and osteoclast-like giant cells. Histopathologically, the tumor was composed of oval to spindle-shaped neoplastic cells with cellular pleomorphism and osteoclast-like giant cells. Immunohistochemically, the tumor cells were positive for vimentin. In addition, the osteoclast-like giant cells were also positive for CD68. Other immunostainings failed to reveal any line of differentiation. The findings were compatible with the diagnosis of undifferentiated pleomorphic sarcoma with giant cells.
Conclusion : Diagnosis of undifferentiated pleomorphic sarcoma by cytological examination alone is difficult. Therefore, immunocytochemical stainings using cell blocks are useful.
