A case of mammary screlosing papilloma (ductal adenoma) interpreted as malignancy owing to the presence of multiple nuclear cytoplasmic inclusions and nuclear grooves

Abstract
Background : Nuclear cytoplasmic inclusions and nuclear grooves are important cytological findings of thyroid papillary carcinoma. Herein, we report a case of mammary screlosing papilloma (ductal adenoma), in which aspiration cytology revealed the presence of multiple intranuclear cytoplasmic inclusions and nuclear grooves, making it difficult to diagnose whether the lesion was a metastatic thyroid papillary carcinoma or malignant breast tumor.
Case : A 60-year-old woman was referred to our hospital because of a mass in her right breast, which was diagnosed as invasive lobular carcinoma. Imaging studies also revealed two small nodules in the left breast. Aspiration cytology of both nodules suggested malignancy : either metastatic thyroid papillary carcinoma or malignant breast tumor. Evaluation of the needle biopsy and excisional biopsy specimens led to a histological diagnosis of screlosing papilloma (ductal adenoma).
Conclusion : Presence of intranuclear cytoplasmic inclusions detected on aspiration cytology of breast tumors may suggest benignity, although clinico-radiological-pathological correlation is always required to make an accurate diagnosis.
