A Study on Changes of Home Medical Intervention Including End-of-Life Care for the Elderly in an Emergency and Critical Care Center

Abstract

 One of the solutions to the exit problem of hospitalized patients in acute care settings and the provision of healthcare that respects the wishes of patients and their families is intervention in home healthcare, including end-of-life care. This report examines the state of emergency centers and home healthcare. Patients aged 65 years or older admitted to our emergency center between 1 April 2018 and 31 July 2021 were examined using electronic medical record descriptions about their outcomes and length of stay. Home visit interventions increased year by year from 12 patients in 2018 to 16 and 27 patients, and the home end-of-life care policy was 2-3 patients in 2018 and 2019, but 7 patients in 2020. The median length of stay was 6 days (3-10 days) for all inpatients, 13 days (8-17 days) for home visit interventions and 12 days (6-23 days) for transfers, but the longest stay was 53 days for home visit interventions, while transfers were scattered patients over 100 days, and death patient was 333 days. In cases where transfer or discharge to a facility is difficult, coordination of home care, including end-of-life care, may reduce the number of cases of long-term emergency center admissions.
