Abstract

We performed transoral partial laryngopharyngectomy for T1, T2 and selected T3 supraglottic and hypopharyngeal cancer using a distending laryngoscope, rigid videoendoscope and electrocautery designed for laparoscopic surgery. This system gave us a wide working space facilitating bimanual manipulation and a wide view of operative area. To date, 22 patients have undergone transoral partial laryngopharyngectomy. For node positive cases, neck dissections were also performed. Although the mean observation period is still short (25 months), the laryngeal preservation rate is excellent (94%) and only one cause specific death was observed. Transoral resection of selected supraglottic and hypopharyngeal cancer could reduce the risk of complications and surgical invasions. This procedure could also maintain good postoperative swallowing function.
