Efficacy of low-dose steroid in a patient with pulmonary sarcoidosis

Abstract

64歳女性．胸部異常陰影の精査目的にX年12月に当科を受診した．胸部高分解能CTでは両側気管支血管束の肥厚と肺野に粒状影を認め，気管支鏡検査では，気管支肺胞洗浄液中のリンパ球分画増加と経気管支肺生検での非乾酪性類上皮細胞肉芽腫が証明され，サルコイドーシス（組織診断群）と診断した．経過観察中に咳嗽の悪化とともに肺野病変の悪化と努力肺活量（FVC）の低下を認め，プレドニゾロン10 mg／日の低用量で治療を開始したところ，肺野陰影は著明に改善し，FVCは増加した．プレドニゾロンは開始後1年1 ヵ月後に終了したが現在まで肺病変の再燃は認めていない．軽症で線維化所見の少ない肺サルコイドーシスにおいても低用量のステロイド治療が呼吸機能改善に寄与する可能性がある．
