Difficult-to-diagnose concurrent onset of sarcoidosis and Hodgkin lymphoma

Abstract

症例は70歳，男性．シェーグレン症候群にて通院中．2 ヵ月前からの食思不振，発熱，体重減少，全身痛を認め，当院紹介となった．造影CTにて左鎖骨上窩・縦隔・腹腔内リンパ節腫脹，頸胸椎に骨破壊を伴う軟部組織腫瘤を認めた．頸部リンパ節生検および経気管支生検・縦隔リンパ節生検にていずれも非乾酪性類上皮細胞肉芽腫が検出され，眼科にて右眼にぶ どう膜炎所見の出現を認めた．一方，18F-FDG PET／CTでは全身リンパ節や軟部組織腫瘤および骨病変に高集積を示し，集積のあった左腸骨からのCTガイド下生検にて類上皮細胞肉芽腫に加え，CD30陽性の異型細胞の浸潤を認めた．頸部リンパ 節検体を再検討し，同様の異型細胞がわずかに同定された．以上より，ホジキンリンパ腫およびサルコイドーシスの合併と診断した．化学療法にて症状は消失し，いずれの病変も軽快した．同時期に発症し，ホジキンリンパ腫に伴うサルコイド反応と鑑別を苦慮した症例であり，報告する．
