Laparoscopic Surgery for Hydrocele of the Canal of Nuck With Intra-abdominal Cyst: Two Case Reports

Abstract

We present two cases of hydrocele of the canal of Nuck with intra-abdominal cyst in girls. Case 1 involved a 1-year-old girl in whom a bulge in the right inguinal region was identified. We diagnosed her as having right inguinal hernia and performed laparoscopic percutaneous extraperitoneal closure (LPEC). We inserted a 3 mm trocar at the umbilicus and found an intra-abdominal cyst connected with the right internal inguinal ring. Hydrocele of the canal of Nuck was considered to have prolapsed into the peritoneal cavity. Nonabsorbable sutures were passed around the internal inguinal ring in accordance with the LPEC, and the cyst was tapped percutaneously. We slightly extended the umbilical incision and inserted a 3 mm trocar close to the first trocar. We then resected the cyst and tied the internal inguinal ring extracorporeally. Case 2 involved a 1-year-old girl. We diagnosed her as having hydrocele hernia of the right canal of Nuck and performed LPEC. As in Case 1, we found an intra-abdominal cyst connected with the right internal inguinal ring. We pulled the cyst through a loop of the LPEC needle to perform a resection. LPEC is being utilized in an increasing number of facilities for cases such as these; therefore, when this procedure is performed for hydrocele of the canal of Nuck, the possibility of intra-abdominal cyst should be kept in mind.
