Case of Resection for Tiny Pulmonary Metastasis of Nephroblastoma Using Preoperative CT-Guided Hook-Wire Localization

Abstract
We herein report a case of resection for pulmonary metastasis using preoperative CT-guided wire localization. A three-year-old female who had a giant abdominal mass was referred to our hospital. The patient was diagnosed as having nephroblastoma with metastases in the bilateral lungs and bone marrow. We first performed nephrectomy of the primary tumor. After chemotherapy coupled with radiation therapy, the metastasis in the bone marrow disappeared; however, the pulmonary metastasis remained as a tiny solitary nodule. Therefore, we planned to perform CT-guided wire localization to mark the tiny nodule. Pulmonary wedge resection was immediately performed by video-assisted thoracoscopic surgery. The nodules could not be detected during the operation by visual inspection. CT-guided localization for tiny pulmonary nodules appears to be a beneficial procedure.
