Extrarenal nephroblastoma is a rare tumor that develops in the retroperitoneal region and presents as a solid mass. We report a case of extrarenal nephroblastoma, including a review of the literature. The patient was an 8-year-old boy who presented with complaints of low back pain, abdominal pain, and fever. Blood tests showed inflammatory findings, and abdominal computed tomography showed a cystic lesion 3 cm in diameter on the anterior surface of the sacral bone, which had no contrast effect. Therefore, retroperitoneal malformation causing infection or rupture was suspected. No findings suggesting elevated levels of tumor markers or metastasis on the image were observed. Tumor resection was performed. The tumor was covered with a capsule, and adhesion with surrounding tissues was remarkable on one side. Histopathological findings revealed extrarenal nephroblastoma, which should also be considered for differentiating retroperitoneal lesions.
