A 3-year-old boy was delivered at 26 weeks of gestation weighing 926 g. At 5 months of age, he was referred to us because of a suspected inguinal hernia. Ultrasonography revealed a bladder hernia. At 1 year and 3 months of age, swelling was observed in the groin area, but ultrasound examination revealed that the bladder hernia had disappeared, and only a small intestine herniation was confirmed. On the basis of his history of bladder hernia and the ultrasonography finding that the processus vaginalis was closed, a direct inguinal hernia was suspected before the operation. At 3 years and 10 months of age, a laparoscopic examination revealed a left direct inguinal hernia, and surgery was performed safely by iliopubic tract repair. The laparoscopic procedure made it easy to identify correctly the area with abdominal wall weakness.
