The number of breast reconstructions with silicone breast implants (SBIs) has rapidly increased in Japan after the operation became covered by health insurance in 2013. However, concavity at the edge of the SBI is conspicuous, especially in patients with less subcutaneous tissue. Fat injection is often added to improve it, but this does not always succeed due to partial fat resorption after injection. In order to improve fat resorption, we transplanted dermal fat grafts (DFGs) for 29 patients with concavity from October 2014 to March 2017, and succeeded in correcting the contour. We use two methods for harvesting grafts. First is harvesting from the edge of the breast scar (DFG-1) , and the other is harvesting from a distant donor site (DFG-2) . Fat resorption or induration was little and the contour did not change 1 year after the operation. Dermal fat grafting is an easy and safe technique that does not require specialized devices. We believe that our hybrid-type breast reconstruction using both SBI and DFG is useful for improving the appearance.
