[Background] After bilateral mastectomy, reconstruction with implants is generally performed. Bilateral breast reconstruction often leads to a symmetrical appearance, but poor outcomes have been reported. As such, we retrospectively analyzed our cases to investigate which factor is directly related to the cosmetic outcome. [Method] Thirty-two cases of bilateral breast reconstruction with breast implants were included in this study. Three plastic surgeons scored each case independently and the mode value was used to represent the cosmetic outcome. Several factors, such as differences in surgical procedure, were statistically analyzed. [Results] An asymmetric surgical procedure of mastectomy was related to a poorer aesthetic outcome (p=0.027) . [Conclusion] Better cosmetic outcomes of bilateral breast reconstruction require symmetry of mastectomy, including the amount and shape of skin excision.
