Abstract
In the three year period Between 1986 and 1989, 19 patients had surgical correction of aneurysms of the tranverse aortic arch with assistance of selective cerebral perfusion (SCP) in our institution. The causes of aneurysms were dissection (type A) in 16 patients and arteriosclerosis in 3 patients. SCP performed with cerebral blood flow 10ml/kg/min at moderate hypothermia (rectal temp. 25°C) and perfusion sites were both inominate and left carotid arteries. 11 patients of 19 underwent open distal anastmosis technique under systemic low flow perfusion at 22°C. The average SCP time was 81.1±27.1 minutes. There was one early death (5.2%) due to bleeding in this series. There were no cerebral complications, but pulmonary insufficiency in 4 cases, renal dysfunction in 5, liver dysfunction in 5. The present data suggests that SCP is useful method for cerebral protection during repair of aortic arch aneurysm.
