Investigation of dialysis patients who developed toxic symptoms of carbamazepine

Abstract

【Case 1】A 65-year-old man was prescribed 400mg of carbamazepine for trigeminal neuralgia. The patient became wobbly and dizzy after 4 days. His serum carbamazepine level was 8.8μg/mL. After pre-dilution online hemodiafiltration (HDF) was performed, the patient showed a marked improvement and his carbamazepine level decreased to 6.8μg/mL. 【Case 2】 A 62-year-old man was referred to the emergency department of our hospital because of difficulty with his body movements, complaining of symptoms such as dizziness, nausea, and bradycardia. The patient had been prescribed 400mg of carbamazepine after undergoing gamma knife. His serum carbamazepine level was 15.8μg/mL. After hemodialysis (HD), the patient showed a marked improvement, and his carbamazepine level decreased to 8.8μg/mL. Toxic symptoms may appear even at regular doses in CBZ. In the present case, we hypothesized that the blood purification such as HD, pre-dilution online HDF would be effective for the removal of carbamazepine.
