A case of clavicular osteomyelitis in a patient on hemodialysis

Abstract
A 43-year-old male with diabetic nephropathy, who had been on maintenance hemodialysis for 6 years, developed precordial swelling followed by fever, which had lasted for a month. On admission, the right clavicle exhibited high-intensity on T2-weighted magnetic resonance imaging, which supported a diagnosis of clavicular osteomyelitis. A contrast-enhanced computed tomography image obtained on day 11 showed bone destruction and an abscess in the thorax and around the right clavicle. Methicillin-sensitive Staphylococcus aureus was detected in the clavicular abscess. Sequestrectomy and debridement were performed on hospital day 57, and septic arthritis of the sternoclavicular joint was observed during the operation. In this case, bacteremia caused by an infected burn apparently progressed to septic sternoclavicular joint arthritis, leading to osteomyelitis of the clavicle.
