We report a case of chronic renal failure complicated by adrenocortical carcinoma. A tumor in the upper part of the left kidney was incidentally discovered by abominal CT scanning in a patient who had been undergoing chronic hemodialysis for 21 years.
Left total nephrectomy was performed. The tumor was 8×7×4.5cm in diameter and was proved to be of adrenocortical origin by positive staining for 3β-hydroxysteroid dehydrogenase. Further pathological findings satisfied 5 out of 9 items in the criteria described by Weiss and showed positive Vimentin staining, leading to the diagnosis of adrenocortical carcinoma. The patient died of sepsis unrelated to adrenalectomy 2 months postoperatively.
Adrenocortical carcinoma in a patient on chronic hemodialysis is very rare in Japan.
