Abstract
Peri-operative outcomes as well as continence rate after surgery were evaluated comparing bilateral interfascial, hemi interfascial and bilateral extrafascial approaches through mini-laparotomy radical prostatectomy.
There were no statistical differences in age, PSA value, biopsy Gleason score and clinical stage between 11, 10 and 24 each cases of bilateral interfascial, hemi interfascial and bilateral extrafascial radical prostatectomies. Average OR time, EBL, pathological T stage and positive margin rate were similar in all surgeries. Urinary continence was recovered by 73%, 80% and 28% in 3 months, and 90%, 91% and 63% in 6 months by bilateral interhemi inter- and bilateral extrafascial prostatectomies, respectively. Biochemical progression free rates during 1 to 24 months(median 12 months)observation period were 0%, 0% and 8%, respectively.
Fascial preservation surrounding prostatic gland could be arranged depending on the anatomical location and the extension level of cancer foci. Even one side preservation of endopelvic fascia may contribute early recovery of urinary continence after radical prostatectomy.
