Abstract
We analyzed the clinical outcome of patients treated by laparoscopic radical prostatectomy (LRP) in Kitasato University Hospital. From 2000 to 2009, 330 patients with clinically localized prostate cancer were treated with LRP by seven surgeons. Of these, 326 cases were carried out laparoscopically. The median operative time was 284 minutes, and the estimated blood loss was 400 ml. There were 22 (6.7%) intraoperative complications, including 6 (1.8%) rectal injuries. The overall positive surgical margin rate was 34.5%. The overall biochemical recurrence-free survival rate was 76.7% at 5 years. Multivariate Cox model analysis showed that extracapsular extension and a positive margin were significant independent predictors of biochemical recurrence. Although the positive margin is relatively high, the clinical outcome of LRP during 10 years is comparable with standard prostatectomy. We should establish an educational system for the next generation to shorten the learning curve.
