A study on the false negative cases in further gastro-endoscopy examinations for gastric cancer mass screening

Abstract
We studied cases that showed false-negative results on further gastro-endoscopy examinations for gastric cancer mass screening from 2003 through 2006 by using data from the regional cancer registry of Fukui Prefecture, Japan. Those screenings included 120,320 individuals and 15,380 (12.8%) were checked up.
Of the individuals who were checked up, 11,643 (75.7%) underwent farther examinations.
Cases with false-negative results were defined as those who had undergone further examinations for suspected diseases other than gastric cancer; however, they were finally diagnosed with gastric cancer.
Further gastro-endoscopy examinations were reviewed in 152 cases.
Among the cases that underwent follow-up, 12 showed false-negative results within 1 year and 44 showed such results within 3 years. The sensitivity of further gastro-endoscopy examinations was 92.7% and 77.6% at 1 and 3 years, respectively.
The ratio of the false negative cases in which the examination results matched with radiography screening findings and the cancer site, was as follows: the upper portion of the stomach, 25.0%; middle stomach, 81.8%; and lower stomach, 41.7%.
The percentage for the middle stomach was the highest.
In two advanced cancer cases with false-negative results, the masses were located in the middle stomach but were not localized. These lesions had been checked at the preceding radiography screening tests.
When referring to preceding radiography screenings, clinicians should carefully examine the indicated lesions, especially those in the middle stomach, while performing further gastro-endoscopy examinations.
