Management Strategy of Colon Polyps Depicted by CT Colonography: An Observational Study

Abstract
Aim: This study aimed to clarify management strategy for patients with colon polyps detected by computed tomographic colonography (CTC).
Methods: This study included 539 patients (223 men and 316 women with a mean age of 69.7 years) who underwent CTC for a positive fecal immunochemical test or gastrointestinal symptom. An incomplete examination rate was calculated by dividing the number of incomplete examinations by the total number of examinations. The diagnostic performance of CTC was calculated based on the number of patients who underwent colonoscopy for abnormal findings of CTC within 1 year after CTC.
Results: The incomplete examination rate was 1.1% (6/539). A total of 102 patients underwent colonoscopy within 1 year after CTC. Among the patients with polyps measuring 10 mm in CTC, 90% (38/42) underwent colonoscopy. In a per-patient analysis, the sensitivity for polyps ≥10 mm was 92% (35/38), and the specificity was 89% (57/64). In a per-lesion analysis, the sensitivity was 88% (44/50), and the positive predictive value was 86% (44/51). In patients with 6-9 mm polyps depicted in CTC, 18% (5/28) had mucosal cancer (Tis) or high-grade adenoma.
Conclusion: The diagnostic performance was high in the real clinical practice, equivalent to reputable clinical trials. The patients with polyps measuring ≥6 mm in CTC were indicated for colonoscopy.
