Abstract

Background and Purpose

Gastric X-ray examination difficulty varies depending on gastric shape (hereafter referred to as gastric morphology). In non-hooked stomachs, barium leakage outside the stomach and poor mucosal adhesion of the contrast agent are more likely to occur, narrowing the range of mucosal surface visualization. This tendency is particularly pronounced in prone double-contrast imaging. We investigated the background factors influencing gastric morphology.

Subjects and Methods

We randomly selected 500 participants from individuals who underwent gastric X-ray examinations at our center in fiscal year 2021. We examined the relationships between gastric morphology and age, sex, body mass index (BMI), and waist circumference.

Results

Non-hooked stomach was observed in 118 cases (23.6%) overall, with a higher prevalence among male, elderly, and high-BMI patients. Multivariate analysis accounting for confounding factors suggested associations with age, sex, and BMI. Among males, only age showed a significant difference, while no significant differences were observed in females.

Conclusion

Physical determinants of gastric morphology were more prominent in males, followed by elevated BMI and advancing age. In contrast, gastric morphology in females could not be reliably predicted based on physical factors alone. These findings suggest that visceral adiposity and aging in males play key roles in the development of gastric morphology. The present study may contribute to improving the efficiency of gastric X-ray examinations and to enhancing the quality of gastric cancer screening.
