Laparoscopic cystectomy for mature cystic teratoma is widely performed, but there is no certainty about the risk of developing chemical peritonitis due to intraoperative spillage. We retrospectively reviewed 148 cases of laparoscopic ovarian cystectomy for mature cystic teratoma in five years (2016-2020) in our hospital. The intraoperative spillage rate was 76.3%, and the incidence of chemical peritonitis was 2.7%. In cases of intraoperative spillage, chemical peritonitis occurred more frequently in patients whose tumor contents spread to the upper abdomen, even after adequate washing. We conclude that it is important to prevent the leakage of tumor contents into the upper abdomen because it affects the development of chemical peritonitis.
