Laparoscopic aspiration of bilateral ovarian torsion due to severe ovarian hyperstimulation syndrome with triplet gestation: A case report

Abstract

 In this report, we report a case of bilateral ovarian torsion due to severe ovarian hyperstimulation syndrome (OHSS) at five weeks gestational age for a triplet gestation at a nearby clinic. A 24-year-old woman became pregnant by timed sexual intercourse after follicle stimulation with gonadotropin but developed severe OHSS. She was referred and admitted to our hospital the same day. On the ninth day of hospitalization, she experienced acute abdominal pain and was diagnosed with ovarian torsion. Laparoscopic findings revealed bilateral ovarian torsion and extremely fragile and hemorrhagic ovaries. Aspiration was performed on the ovarian cysts for volume reduction, which made it easier to perform detorsion without causing accidental bleeding. The postoperative course was uneventful.

 Therefore, we concluded that the laparoscopic aspiration of ovarian cysts may be considered in the case of ovarian torsion by OHSS during pregnancy.
