Abstract
Six cases of recurrent dislocations after cementless total hip arthroplasty are reported.
Three patients were treated surgically and the other three cases were observed conservatively.
Revision operations were done for three hips by replacement with elevated liners, long necks, and 26mm heads, to obtain good stability of hip joints and appropriate tension of soft tissues.
As a result, the mean lateral offset was increased from 5mm to 8mm, and the discrepancy of length of the lower extremities was changed from 5mm to 11mm.
According to the surgical findings, the soft tissues were remarkably loose in all three cases, and impingement between soft tissues and bone was seen in two cases.
It was suggested that a shorter offset and closer position of the femoral head to the greater trochanter might leave soft tissue loose around the hip joint, and raise the risk of recurrent dislocation after cementless TKA.
