Abstract
Recentry, medical treatment for schizophrenia has been made mainly as outclinic way from the former hospitalization, and the importance has been transferred to the community-care. In our Karasuyama Mental Hospital affiliated with Showa University, psychiatric rehabilitation therapy has been made for the patients with schizophrenia for over 25 years, and constant results have been obtained, however, there are many problems for the treatment of schizophrenia. Especially, the problems at the initial stage of schizophrenia should be reviewed from the viewpoint of prognosis for the long term including the treatment and therapy of schizophrenia hereafter. Accordingly, we have surveyed and investigated schizophrenia in 55 cases showing its occurrence with 5 years hospitalized in 1981 and 1982, 73 cases of the same criteria in 1972 (10 years ago) and 72 cases in 1962 (20 years ago) for application to the changes in years. Thus we have obtained the results as followed: 1) With the yearly change, early admitting and early discharging tendency has been clarified for the group of initial schizophrenia, and this tend was confirmed to show better results for the social adaptability. 2) In spite of this fact, it is considered that the prognosis in view of the long term disease of schizophrenia would not be changed greatly. 3) The effect of at-home medicare will be far better than the hospitalized treatment, the influence of environment mainly with social life would be far greater. 4) Hereafter, the short-term hospitalization and at-home treatment of schizophrenia will tend to be further increasing, and the changes in hospitalizing treating forms can be estimated. 5) By the present study, an important suggestion might be given for the management of mental hospital.
