Abstract

The update of 2018 AHA/ASA stroke guidelines (2018 Guidelines for the early management of patients with acute ischemic stroke) has been published in January 2018. According to many updates and deletions, the most impact was seen in brain imaging and mechanical thrombectomy section. Lead by two RCTs (DAWN · DEFUSE 3), the time window has expanded in mechanical thrombectomy which indicated the future prospective in acute ischemic stroke treatment as “time dependent” to “tissue dependent”. The concept is idealistic but unfortunately the mismatch software is not affordable in Japan. This symposium will refer to the its blind spots and discuss about the reason why current version of 2018 AHA/ASA stroke guidelines has made several changes in few months after the original publication.
