We performed MRI–guided focused ultrasound (MRgFUS) unilateral globus pallidus internus (GPi) pallidotomy for 3 Parkinson disease (PD) patients with motor fluctuation and dyskinesia (aged 59 to 78 years, all women). The improvement of motor function in 2 patients and dyskinesia in all patients continued up to 1 year after the procedure. Especially in 1 patient, dyskinesia disappeared completely. Adverse events were mild and transient. Further study with more patients and longer periods is necessary ; however, MRgFUS unilateral GPi pallidotomy may become one of the viable therapeutic options for advanced PD.
