Acute incidental cerebral microinfarct (aiCMI) has recently being acknowledged, and is found in memory clinics. Most aiCMIs have atherosclerotic origin, and have close relation with vascular dementia (VD)/ white matter disease. We found 1% out of 1831 patients who visited our MRI room, mostly older individuals, which seems fewer than those in the previous reports (3–13%). Since VD is often accompanied by Alzheimer's disease, we should pay attention to aiCMI and VD, in order to maximize patients’ quality of life. Possible management of aiCMI/VD is also described.
