Although the diagnosis of round cell tumors based on morphology is difficult, a detailed evaluation of the histomorphology of HE specimens after gathering sufficient clinical and imaging information can bring the diagnosis considerably closer at the stage prior to immunostaining and genetic testing. Narrowing down the diagnosis reduces unnecessary testing and avoids pitfalls in immunostaining. This article explains the diagnostic process for round cell tumors in the following order:exclusion of non-sarcomas, differential diagnosis of round cell sarcomas, immunostaining and its pitfalls, and genetic testing.
