Abstract
Root Cause Analysis is an easy and preferable tool in healthcare. (1) Call inter-disciplinary experts from the frontline services and manage their time. Draw a flow chart in chronological order in which the event is separated to several stages. (2) Continually dig deeper by asking why and why questions using brainstorming, analyze underlying cause and effect systems, and identify risks. (3) Determine potential improvement in processes or systems, including participation by the leadership of the organization. This process should be as impartial and internally consistent as possible.
