要旨
サルコペニアは,European Working Group on Sarcopenia in Older People(EWGSOP2)で「筋肉量の減少,さらに筋力の低下および/または身体能力の低下」と定義される.サルコペニアの原因は多岐にわたり,加齢に伴う一次性サルコペニア,加齢以外の要因による二次性サルコペニアに大別され,二次性サルコペニアの原因として不活動,疾患,低栄養が主である.サルコペニアの併存は間質性肺疾患(interstitial lung disease: ILD)患者に不良な転帰をもたらす要因として注目されている.ILDにおけるサルコペニアの併存率は,わが国のILD患者で32.1%,ILDの中で最も頻度の高い特発性肺線維症(idiopathic pulmonary fibrosis: IPF)患者では,26%と報告されている.サルコペニアは,栄養障害に起因することも多く,ILD患者における栄養障害の原因は,呼吸筋などの代謝亢進,全身性炎症,低酸素血症,呼吸困難による食事摂取困難,不活動,抗線維化薬による副作用などがあげられているが,現時点では詳細不明である.十分な栄養管理下での運動療法がILD患者には重要であると考えられる.

サルコペニア
EWGSOPでサルコペニアは,骨格筋疾患(筋障害)と考えられ,骨格筋量の加齢に伴う低下に加えて,「筋肉量の減少,さらに筋力の低下および/または身体能力の低下」と定義される1).サルコペニアの診断は,人種や地域差なども考慮されるため様々な診断基準が用いられている.本邦においてはAsian Working Group for Sarcopenia: AWGS 20192)による診断アルゴリズムを使用することが推奨されていた.AWGSはサルコペニア診断に関する新たな指針を2025年11月に発表し(AWGS 2025),サルコペニアを「加齢に伴う骨格筋量の減少と筋力の低下」と定義した.また,今後のサルコペニア診断においては,AWGS 2019に代わり,AWGS 2025の使用が推奨されている.
サルコペニアの原因は多岐にわたり,加齢に伴う一次性サルコペニア,加齢以外の要因による二次性サルコペニアに大別され,二次性サルコペニアの原因として不活動,疾患(侵襲,臓器不全,炎症性疾患,内分泌疾患,悪液質など),低栄養が主であるとされる1).サルコペニアの有病率は,65歳以上の日本人地域高齢者1,851人(女性50.5%,平均年齢72.0±5.9歳)を対象にした研究で男性:11.5%,女性:16.7%と報告されている3).また,慢性呼吸器疾患の代表である慢性閉塞性肺疾患(chronic obstructive pulmonary disease: COPD)を対象とした研究では27%4)であったと報告されている.慢性呼吸器疾患におけるサルコペニアの併存は,転倒リスクや骨折率の上昇,日常生活活動(activities of daily living: ADL)の低下,認知機能の低下,摂食嚥下機能の低下,耐糖能の低下,死亡率の上昇など様々なリスクが高まる重大な問題である5).
また近年では呼吸サルコペニアという概念も提唱されており,注目されている6).呼吸サルコペニアは,呼吸筋力低下と呼吸筋量減少が示唆される病態と定義されている6).呼吸筋量の評価方法は,超音波診断検査や胸部Computed Tomography(CT)を用いて測定されるが,呼吸サルコペニアの診断において呼吸筋量の低下を示す明確な基準値はなく,現時点では呼吸筋量減少を定義するデータは不足しており,呼吸サルコペニアを確定診断することは困難である7).
ILDとサルコペニア
間質性肺疾患(interstitial lung disease: ILD)におけるサルコペニアの有病率に関する研究ではCOPDより高い傾向がある.ILD患者のサルコペニアの有病率は,いくつか報告されており日本人を対象とした研究では32.1%8),ILDの中で最も頻度の高い特発性肺線維症(idiopathic pulmonary fibrosis: IPF)患者では,39.3%9),22.9%10),37.5%11),26%12)であった.当院のILD患者においても31%でサルコペニアが認められた(表1・図1).ただし,サルコペニアの診断基準が報告によって異なっていた.また,重症例で身体機能の検査が困難な患者が含まれていなかった.さらにLi Jら12)は,有病率は研究対象者の背景(環境)によって異なり,地域住民よりも入院患者,急性期後ケア施設,介護施設入所者でより患者数が多く,これらのことが考慮されていないためILD患者のサルコペニアの有病率は,過小評価されている可能性を指摘している.今後は,多施設での大規模な調査が必要と思われる.

サルコペニアは,ILD患者における死亡の独立した危険因子である13).ILDにサルコペニアを合併すると患者報告アウトカム(patient-reported outcomes:PRO)や抑うつに影響を与え,運動耐容能や生活の質(quality of life: QOL)が低下する9,14).また,IPF患者では,脊柱起立筋の筋横断面積の低下15,16),fat-free mass index(FFMI)の低下17),低body mass index(BMI)18)は予後不良因子である.
現時点でILD患者における骨格筋量の減少の詳細な原因は不明であるが,呼吸筋などの代謝亢進,全身性炎症,酸化ストレス,呼吸困難による食事摂取困難,不活動,抗線維化薬による副作用など多因子の関与があげられている(図2)19).抗線維化薬であるニンテダニブとピルフェニドンは肺機能の低下を抑え,疾患の進行を抑制することや,急性増悪を抑制する作用がある.しかしながら,どちらの薬剤も消化器に関連した副作用が高頻度にみられ,その割合はニンテダニブでは下痢(56%),悪心(22%),嘔吐(11%)と体重減少(5~10%),ピルフェニドンでは食思不振(23%),胃不快感(14%),嘔気(12%)と下痢,胸やけ(5%以上)である.これらの有害事象は,エネルギー摂取量や消化吸収に影響を与え,骨格筋に負の連鎖をもたらすことが考えられ適切な管理が必要である.
ILDの栄養障害
ILD患者は,エネルギーの需給バランスが崩れ栄養障害を引き起こす.要因として,疾患の進行に伴う呼吸仕事量の増大・全身性炎症や酸化ストレスなどによるエネルギー消費量の増大,呼吸困難や咳嗽・抗線維化薬の副作用などによる食事摂取量の低下に伴うエネルギー摂取量の減少があげられる.ILDの栄養障害の頻度については,IPF患者で18.5~55%と報告20,21,22)されている.この幅は評価方法の違いと考えられる.当院では,170例のIPF患者を対象にControlling Nutritional Status Scoreを用いて調査したところ,軽度~中等度の栄養障害を40%に認め,且つ栄養障害は予後不良因子であった22).その他,生命予後と関連する因子として,BMI18,23),経時的な体重減少18,23,24),Geriatric Nutritional Risk Index(GNRI)25)が報告されている.したがって栄養状態のモニタリングと早期の栄養介入が重要と考える.一方でILDに対する栄養療法についての報告は少なく,特発性胸膜肺胞線維弾性症に対する早い段階での高脂肪・高タンパク食を中心とした栄養指導は,体重減少の進行を抑えると報告されている26).また,Yavari M, et al.は,IPF患者にビタミンD,C,Eを組み合わせて3か月間補給すると,呼吸機能にプラスの影響を与え,炎症と酸化ストレスが軽減される可能性があると報告している27).
地域高齢者やCOPDにおいては,栄養療法は効果的な介入であることが示されている.COPD患者に対する栄養療法は,筋力と運動耐容能に有益であるとされている28).また,サルコペニアの高齢者を対象に行われたランダム化比較試験では,ビタミンDおよびロイシン強化ホエイプロテイン経口栄養補給剤を13週間摂取した結果,対照群に比べ四肢骨格筋量と下肢機能が改善したことが示されている29).他にも,高齢者を対象にした研究では,抗炎症作用と抗酸化作用の両方を持つアスタキサンチンは運動トレーニングとの組み合わせでプラセボ群より筋力,筋横断面積,持久力を向上させる効果があることが示唆されている30).これらは,ILDにおいても効果が期待できる.
サルコペニアに対する治療戦略
サルコペニアの治療戦略としては,栄養療法と運動療法の併用が重要で31),十分な栄養管理下での運動療法を行う必要がある.サルコペニア診療ガイドラインにおいて,サルコペニアを有する人へのレジスタンストレーニングを含む包括的運動介入と栄養療法による複合介入は,単独介入に比べサルコペニアの改善に有効であり,推奨される32)とされている.
サルコペニアの栄養管理には,十分なカロリーとタンパク質の摂取が必要であり,これらが不十分な場合,サルコペニアの改善は困難である33).しかしながら,ILD患者は,前述したように呼吸困難や咳嗽,抗線維化薬の副作用などにより食事摂取困難を来しやすい.そのため,食事を工夫し効率の良いエネルギー摂取が必要で,脂質や多脂食品の利用,少量頻回食の導入,栄養補助食品の活用などが有用である34,35).ただ,脂質過多となると消化不良などの可能性があるので注意が必要である.嗜好や生活リズム,体調などに見合った食品選択をすることなどが重要である35).食事摂取が困難な症例は,その原因検索も重要であり,前述した理由だけでなく,疲労感,独居,味覚障害なども食事摂取量に影響する.そのため十分な問診を行い,多職種で協力し介入していくことが重要である.また,見落としがちなのが嚥下障害であり,軽症例でも嚥下障害が報告されている36)ため注意が必要である.
サルコペニアを有する高齢者に対する運動療法は,四肢骨格筋量,筋力および身体機能の改善に効果がある6,32,37).しかしながら,サルコペニアを有するILD患者に対する運動療法を中心とした包括的呼吸リハビリテーションの報告はきわめて限られている.ILD患者に対する包括的呼吸リハビリテーションは息切れの軽減,運動耐容能の改善,QOLの向上といった効果が認められており有効な治療戦略である38).そのため,サルコペニアを有する症例に対しても効果が期待されるが,さらなるエビデンスの蓄積が必要である.
