Abstract
We report a rare case of aneurysm on double origin of the posterior inferior cerebellar artery (double PICA). A 44-year-old male patient was admitted to our hospital for diabetes mellitus. He complained of sudden headache and a CT scan showed massive subarachnoid hemorrhage. A right vertebral angiogram revealed double PICA of the extracranial lower branch and upper intracranial branch. The right vertebral artey itself appeared to be dissected. We withheld treatments during the acute stage. Twenty days later, we treated the aneurysm by the endovascular technique. After the treatment, the aneurysm was completely packed with GDC coils and the parent artey was saved. Postoperatively, the patient had need of a V-P shunt, and was discharged with no symptoms.
