Abstract
A 39-year-old man developed disturbance of consciousness following abnormal behaviors such as taking another person’s bicycle after surfing. MRI showed infarctions in cerebellar hemispheres and thalamus bilaterally, and MRA showed bilateral vertebral artery dissections. On follow-up MRA, bilateral false lumens of the dissections showed similar signal changes over time due to vessel repair. Vertebral artery dissection is known to occur with minor trauma, and therefore hyperextension of the neck during surfing may have been responsible in this case. Bilateral vertebral artery dissection due to sport is rare and this valuable case is discussed in terms of the disease mechanisms.
