Reversible cerebral vasoconstriction syndrome (RCVS) typically affects the bilateral medium-sized intracerebral arteries and their branches. We describe a woman with RCVS restricted to the ipsilateral hemisphere after carotid artery stenting (CAS). A 74-year-old woman presented with headache and left-sided weakness 17 days after undergoing right CAS. MR imaging showed borderzone infarcts and catheter-based angiography identified new irregularities of the right anterior cerebral and right middle cerebral artery. A follow-up angiography after about a month showed complete resolution of the cerebral arterial vasoconstriction. These results suggest that RCVS may occur after CAS. The mechanism is unclear, however, it may be due to the disturbance of cerebral autoregulation.
