Background and Purpose: The precise survey data of the current status, such as the frequency of comorbidities and risk factors in patients with acute stroke who received rehabilitation in Japan, is unknown. The purpose of this study was to investigate the frequency of comorbidities, risk factors, and rehabilitation costs in patients with acute stroke. Methods: This multi-institutional registry study included six acute care hospitals in Japan. The hospitalized patients diagnosed with acute stroke, including cerebral infarction (CI), cerebral hemorrhage (CH), and subarachnoid hemorrhage (SAH), registered from October 17, 2022 to June 30, 2023 were included. The clinical background such as comorbidities and risk factors, and rehabilitation costs that were claimed and the doctor judged could be claimed based on the clinical background, which was evaluated for each patient. Results: A total of 453 patients with acute stroke (374 with CI, 77 with CH, and 2 with SAH) were enrolled. In the comorbidities, among the patients with acute stroke aged eighty-five years and older, the most common was the non-dialysis-dependent chronic kidney disease in 51 patients (52%), followed by atrial fibrillation in 38 patients (39%) and malnutrition in 37 patients (38%). These diseases were more common than under eighty-five years. The rehabilitation costs for other diseases that can be claimed were the disuse syndrome in 155 patients (34%), followed by cardiovascular disease in 115 patients (25%). Conclusion: Acute stroke patients undergoing rehabilitation have multiple concomitant diseases. Thus, the combined rehabilitation considering them is required.
