Abstract
A 61-year-old man was admitted with acute back pain. Enhanced computed tomography (CT) showed acute type IIIb aortic dissection which had extended across the preexisting infrarenal abdominal aortic aneurysm (AAA). Medical therapy was initially performed in the intensive care unit. However, Severe lower limb ischemia was caused next day, CT showed that the true lumen of abdominal aorta was compressed by false lumen, AAA and bilateral common iliac arteries were not enhanced, due to compression by the occluded false lumen. Thoracic stent graft was placed to close the primary entry tear of the proximal site of the descending aorta, and abdominal stent graft was placed into AAA to improve the perfusion for lower limbs. Postoperative CT revealed that the true lumen of the descending aorta was dilated, though the endoleakage remained a little, and no endoleakage was found in the AAA. He returned home without any complications. This procedure may be an efficacious treatment in case of complicated acute aortic dissection.
