A female patient in her twenties was admitted to our hospital with abdominal pain. Following clinical examination, she was diagnosed with Meckel's diverticulitis and localized peritonitis. The patient underwent conservative therapy. Despite treatment, abdominal pain did not fully resolve. Hence, we performed open surgery on her and discovered that a solid tumor attached to the small intestine was the source of pain. Furthermore, this tumor was connected to the small intestine with string-like connective tissue. The pathological findings revealed leiomyoma, thus we diagnosed her with parasitic leiomyoma. This case report describes the very rare parasitic leiomyoma in detail.
