Recent development of the analysis of vascular anatomy and the microsurgical techniques has greatly expanded the clinical utilization of the free tissue transfer. The availability of the merit of both skin graft and skin flap is the advantage of this technique. The former is the one stage transfer and the latter the thickness and the neutrient vessels. Appropriate flap planning, skilled hands for microvascular anastmosis and careful post-operative observations are all essential for the high taking rate. We have experienced 14 free tissue transfers in the past 15 years in our department and obtained good results on the whole. We presented 3 cases in details and discussed them with some literature review.
