Abstract
Present report deals with the anesthetic results of the controlled epidural anesthesia for cesarean sections performed at Kyorin university Hospital. In which bupivacaine, etidocaine and lidocaine were used to maintain controlled epidural block. Data were collected from operations performed between April 1973 and October 1976. Anesthetics of high concentration (0.5% bupivacaine 75mg) was used on eleven patients for the purpose of studying conceutration effect. to overcome unblocked segments in the lumbo-socral region. Anesthetic was administered through Tuohy needle placed in the caudal direction prior to the insertion of epidural catheter at the 3rd or 4th lumbar interspace. Patients were kept in lateral, head up position to study gravity effect also. For the study of effect, anesthetics of moderate concentration (75mg of etidocaine) were injected to five patients. The initial onset time of bupivacaine was measured around 5-10 minutes and that of etidocaine around 3-6 minutes. Reinforcing doses of the weak solution of local anesthetic was administered to study the volume effects by additional injection of one percent lidocaine (70-100mg) to the cephalad direction through the catheter. There could be observed no toxic reations with the dosage of the agents used in this series. There was no foetal depression as judged by Apgar scores at both one and five minutes. Prescribed anesthetic method is safely administered by trained anesthesiologist.
