Abstract

Objective: In treating dural arteriovenous fistula (DAVF), transvenous embolization (TVE) of the affected sinus is a good option. This method, however, sometimes requires a huge amount of coils to achieve extirpation of shunts. In this report, the usefulness of Penumbra Coil 400 (Penumbra, Inc, Alameda, CA, USA; PC400), which has a larger primary diameter in TVE, is presented.

Case Presentations: Four patients of DAVF (cavernous sinus: 2 cases, transverse sinus: 1, jugular foramen: 1) were treated by TVE from 2013 to 2014. The mean use number of PC 400 in four cases is 7.5 and especially J Soft is used for 87% of the whole. After the treatment, the symptoms of all cases disappeared without neurological complication. No recanalization occurred during the follow-up period.

Conclusion: Primary coil diameter of PC 400 is larger and J Soft is weaker in shape memory than that of the conventional 10 type coils. We are able to obtain the higher volume embolization ratio with less numbers of coils. PC 400 is a useful device for the DAVFs.
