Features of gastric cancer invading SM2 and deeper that were not detected on screening endoscopy

Abstract

Between April 2008 and March 2016, 256 lesions diagnosed as gastric cancer within 2.5 years after screening endoscopy were studied retrospectively. Invasion depth of M/SM1/SM2-3/MP/SS was seen in 193/27/27/6/3 lesions. The 36 gastric cancers invading SM2 and deeper were detected more frequently in the cardia, upper body and middle body than in other areas, especially in the lesser curvature of the cardia and posterior wall of the upper to middle body. These 36 lesions were divided into three groups according to the previous endoscopic findings. The 16 lesions (44%) in the rapidly increasing group could not be diagnosed as cancer on previous screening endoscopy. However, the 9 lesions (25%) in the insufficient observation group and 11 lesions (31%) in the suspicious cancer group could have been detected on previous endoscopy. The quality of endoscopic examinations will improve through evaluation of gastric cancers detected after screening endoscopy.
