Abstract
Local bacterial infection and systemic host defense participate in the initiation and progression of periodontal disease. The influence of stress on host defense has been suggested. Oral health care important in periodontal therapy is affected by self-efficacy. We evaluated the effects of stress and self-efficacy on peri-odontal disease. Subjects were 36 periodontitis patients in the maintenance phase-21 men and 15 women with a mean age of 62.6 years. We studied scales of profile of mood states (POMS) , saliva chromogranin A (CgA) concentration, and self-efficacy score. Patients were divided into stress and non stress groups based on the POMS scale. Probing depth (PD) , bleeding on probing (BOP) , and O′Leary′s plaque control record (PCR) measured at first visit, final examination, and maintenance phase were used for analysis. The mean saliva CgA concentration was significantly higher in the stress group than in the non stress group. The score of self-efficacy scale was significantly higher in the non stress group than in the stress group (P<0.001) . Average PD at the maintenance phase was higher in the stress group in the non stress group. Correlations were seen between saliva CgA concentration and average PD or percent of BOP positive sites at the first visit. PCR in the group with high self-efficacy at the maintenance phase was significantly lower than in the group scorting low self-efficacy. These results suggest that those suffering stress are easily to affected by or suffer a recurrence of periodontal disease, and oral cleaning influenced by self-efficacy may affect the progression of periodontal disease.
