We report a patient with extensive severe chronic periodontitis who underwent an initial preparation, periodontal surgical treatment, oral-functional recovery treatment using fixed prosthetic appliances, and supportive periodontal therapy (SPT). As a result of this treatment, favorable stomatognathic function has been maintained for 21 years. The present case suggests that for the long-term stability of fixed prosthetic treatments in patients with severe periodontitis, the maintenance of favorable oral hygiene conditions, the stability of the patient's life environment, a favorable positional relationship of the abuting teeth, the production of fixed prosthetic appliances applied to the inner crowns, the maintenance of an occlusal relationship enabling a symmetrical masticatory movement, the continuation of SPT, and dealing with intra-oral changes appropriately are important.
