Usefulness of combined periodontally accelerated osteogenic orthodontics and periodontal regenerative therapy in patients with periodontitis

Abstract

Periodontally accelerated osteogenic orthodontics (PAOO), a procedure for accelerated orthodontic treatment, allows the bone enlargement in addition to shortening the period of orthodontic treatment. Although the clinical benefit of PAOO has been reported in many periodontally healthy subjects, there are few reports of its application in patients with periodontitis.

The purpose of this retrospective cohort study was to verify the clinical usefulness of combined PAOO and periodontal tissue regenerative therapy in patients with periodontitis. The 48 subjects enrolled in the study consisted of 37 patients with periodontitis and 11 periodontally healthy patients who underwent PAOO treatment between 2007 and 2022. The periodontitis patients were subdivided into 4 groups (I-IV) based on the new classification of periodontitis. The clinical data of the 5 groups before and after the surgery were compared. There was no significant difference in the number of remaining teeth before and after surgery in any of the groups. The mean periodontal pocket depth was significantly reduced postoperatively even in the Stage III and Stage IV groups (P < 0.01). The periodontal pocket depth and alveolar bone level at the perpendicular bone defect sites were significantly reduced following surgery, even in the Stage III and Stage IV groups (P < 0.01). There was no significant difference in the dynamic period of orthodontic treatment between the two groups, with the treatment completed in approximately a half of the normal treatment period.

This study suggests that PAOO combined with periodontal tissue regeneration therapy is an effective treatment method that can simultaneously and quickly improve periodontal tissue regeneration, bone formation, and dentition, regardless of the severity of periodontitis.
