A 75-year-old female was admitted to our hospital with a pain and swelling in the right lower extremity. CT scan showed a hematoma under the crus and an arterio-venous fistula was considered. She had no history of trauma there. Angiography was performed directly, and revealed an arterio-venous shunting from the peroneal artery. Successful embolization was performed and her symptom disappeared. After 20 days the local skin became necrotic therefore debridement and skin transplantation were required.
