Abstract
A 10 year 9 month old girl showing the mesially tipped mandibular right second molar was referred by her general dentist to the clinic of the pediatric dentistry of the Tsurumi University Dental Hospital. Her mandibular right first molar was extracted, because of the poor prognosis of its root canal treatment and developing the third molar in the same side. After extraction of the first molar, a crown distal-shoe space maintainer was inserted for guiding the second molar. Subsequently, the second primary molar was extracted due to the root resorption, and crown of the second molar was surgically exposed at the same day. After eruption of the premolars, a segmental arch was placed for uprighting the second molar. The second molar was aligned into the position of the first molar extracted. In this case, the acceptable occlusion was obtained with fairly simple appliances.
