Abstract
The results of treatment of 21 patients for postoperative stress ulcer in hepatobiliary and pancreatic disorders were examined by dividing them into those observed before and after the administration of cimetidine (the former and latter periods, respectively) and compared with each other. Further, the therapeutic and preventive effects of cimetidine were studied. For all the patients treated surgically, the rate of those in whom stress ulcer developed was 2.3% in the former period and 4.7% in the latter period, showing a tendency to increase. The mortality was 55.6% in the former and still as high as 41.7% in the latter. When the present disease had its onset after hepatobiliary or pancreatic operations, the condition of the patients became serious, and the rate of efficacy of cimetidine in them was 58.3%, with almost no difference shown from the 57.1% registered in the former period. Among patients with malignant diseases, particularly those accompanied by jaundice more than 2mg/dl, the onset of the present disease was seen in those with PTCD with a frequency of as high as 17.6%. It was thought that prophylactic therapy with, cimetidine was effective against the onset of stress ulcer when it was given to patients with malignant diseases who had preoperatively duplicated complications and who had a history of hematesis or jaundice.
